چکیده
Abstract Background Patients with a suspected early cancer affecting the periampullary region may be offered resection in form of pancreatoduodenectomy (PD) if they are suitable candidate. Whilst this offers some patients potential cure, operation is high-risk and disease recurrence common. who found to have ampullary adenocarcinoma (AA) or cholangiocarcinoma (CC) on their postoperative histology known relatively favourable oncological outcomes margins clear. This study aimed investigate correlation between positive margin both five-year survival. Methods Data was extracted from Recurrence After Whipple's (RAW) study, multicentre retrospective cohort following performed for pancreatic head malignancy (29 centres 8 countries, n=1,484). suggestive AA CC were identified compared one another by status. Five-year survival rates using Fisher's exact test. Results A total 394 had (26.5%) 364 these (92.4%) status recorded. Of these, 295 (81.0%) no (R0), 69 (19.0%) at least (R1) zero (0.0%%) an incomplete (R2). R0 lower (41.4% vs 63.8%, p=0.0012) higher (59.0% 30.4%, p<0.00001) those R1 disease. 205 (13.8%) 202 (98.5%) 113 (55.9%) 86 (42.6%) three (1.5%) (60.2% 76.7%, p=0.015) (38.1% 15.1%, p=0.0004) Conclusions In our majority underwent PD margins. These significantly improved outcomes.
